You go ahead and go ahead and complete that note. So I'm just going to click into the subjective area and go ahead and knock this out real quick. The patient is a 62 year old gentleman who comes in today for a skin check period. He's had two melanomas in the past semicolon one with a Clark level 4, 4.9 millimeter melanoma on the left arm period. There were five mitoses per square millimeter with no ulceration period. This was excised on April 14th 2021 with a negative sentinel lymph node biopsy period. New paragraph on April 23rd 2021. He had another lesion distal to this left arm and this was removed and was shown to be a 1.4 millimeter thick melanoma comma level 4 less than one mitosis per square millimeter with ulceration absent period. New paragraph. He currently notes that he's doing well with no new moles. He points to a wart on the third finger of his left hand that he's had for some time period. He also has one on the right finger between the first and second digit period. New paragraph. Normal derma objective. As you can see I'm just scratch that clicking in between the different sections of the note and then I just go ahead and begin the dictation. As you can see I just made a quick mistake. I had to get that out of there. I said scratch that. There are lots of different included commands with the Dragon Medical One application and we will show you how to use it and how to use all of those commands completely during the training period. So let me go ahead and finish this. One period wart colon on the third finger of the left hand. He had a three to four millimeter hyperkeratotic papule and this was frozen with liquid nitrogen with three brief freeze-thaw cycles period. On the next visit comma we will freeze it a bit harder if it appears to persist and he didn't get too much of an inflammatory response semicolon and also on the third right finger he had a three millimeter papule that was frozen with liquid nitrogen as well period. He had on his left forearm and left wrist two well-healed scar sites with no evidence of pigmentation or nodularity in or around the scar sites semicolon and we will watch for pink lesions that are persistent comma and he will continue self skin checks period. New paragraph. Three period nevus colon. He had a few brown macules and papules scattered on torso and extremities but nothing concerning for malignancy period. We'll see this gentleman back in our office in a month to treat the warts if they are still present and in three months for the next skin check period. Microphone off and it's just that easy.
