If you file a complaint with the FCC, you must include the following information for the FCC to investigate. Your name, address, phone number, and email address. The website URL or name of the program and channel related to your complaint. Date and time you watched the program that had the captioning problems. Device or software you used to view the program. Detailed descriptions of the captioning problem. For example, garbled captioning, missing captions, brief videos or pictures of the problem would be helpful but are not required. Include any additional information that may assist the FCC in processing your complaint. Your preferred method to be contacted, for example, email, by telephone, or by letter. If you choose to file your written complaint directly with the distributor, you may be able to find the contact information on the distributor's website.
