+1 (831) 222-8398Speaker 1: Okay, so for your data collection tool this month, I have for you a referral form. The beginning of the school year is a great time to start collecting those referrals. So I have the print version, which is pretty straightforward, but I want to walk you through the Google Forms digital versions I have for you. So I have one for teachers and one for students self-referrals. Now these referral forms are a great way for teachers to kind of give you some info about the student they're concerned about and for you to have it recorded right there. Now I did give you the paper version, and if you get these back, you could file them in a file cabinet, but I prefer the Google version because when you click that responses tab, it records everything. You can even export it to an Excel sheet and see the student name and a timestamp indicating when they were referred. It's a great way to make sure that no one slips through the cracks. So you're going to send this out to teachers. You click that send button, and you can send it directly to their email, or you can just get a link. So what I would do, you could then turn that link into a QR code that they could scan, or you could just email out the link, or you could link this on your school website or counseling page. This way teachers can quickly make a referral without having to find the sheet and everything. Now if you are going to use the paper forms, I do recommend having them everywhere. So give them to teachers at the beginning of the year, have them in your office, have them in the front office, and have them in the teacher's lounge or by the mailboxes. Now they're going to write their name and the student's name. Then they can click the area that they recommend counseling services that they need. So if the student's struggling with social skills, or maybe it's bullying, self-esteem, they can write if they have a recommendation about individual or group counseling, and then they're going to mark the severity of the issue. Now you can customize this. You can add, delete, or change these categories. You could also have a space, you could add a question that's just a free response if they want to add any other comments, anything like that. Now I also want to share with you the student version. Now this one is more simple than the teacher version, so the kiddos can answer the questions themselves and it's pretty straightforward. So you could even use this with as young as like elementary school, you know, maybe second or third graders. They're just going to write their teacher's name, their name, what they need help with, and then how big is their problem on a scale of one to five. Simple as that. Now you could make this more complex if you want to use it with middle school, high school, or even upper elementary. You could add some boxes where they could write a little more detail about why they want to come see you. Okay guys, I hope you find those referral forms super helpful.
